01Who this agreement covers

This agreement is between the parent(s) or legal guardian(s) named below (“I,” “me,” “Parent”) on behalf of the minor wrestler named below (“Wrestler”), and Capital City Wrestling Club, together with its owners, officers, directors, employees, coaches, contractors, volunteers, chaperones, volunteer drivers, and any affiliated or successor entity (collectively, “CCWC”).

It applies to every CCWC travel event during the season shown above, including tournaments, duals, camps, team trips, and the transportation, lodging, meals, practices, weigh-ins, and free time connected to them (each an “Event”). I do not need to sign a new agreement for each Event; CCWC will send a short trip sheet with dates, destination, hotel, and drivers before each Event, and this agreement governs each of them.

02The risks I am accepting

Wrestling is a full-contact combat sport. I understand that participation carries a real risk of serious injury, permanent disability, and death, and that no amount of coaching, supervision, or equipment removes that risk. The risks include, but are not limited to:

  • Injury from wrestling itself: concussions and other head injuries, neck and spinal injuries, fractures, dislocations, sprains and ligament tears, cauliflower ear, dental injury, cuts, and injuries caused by the actions of an opponent, a training partner, or an official.
  • Skin infections and communicable illness: ringworm, impetigo, staph and MRSA, herpes gladiatorum, and other conditions spread through mat contact and shared spaces such as hotel rooms and vehicles.
  • Weight management and conditioning: dehydration, heat illness, and the physical stress of making weight. CCWC does not endorse unsafe weight cutting; I am responsible for my wrestler's nutrition and hydration practices.
  • Travel: motor-vehicle collisions and other risks of road travel in private vehicles, rented vehicles, or vans driven by coaches, staff, or volunteer parents; risks of air or bus travel when used.
  • Lodging and free time: injuries in hotels, pools, restaurants, and public spaces; periods when wrestlers are not under direct adult observation; theft or loss of personal property.
  • Venue conditions: crowded arenas, equipment and mat conditions controlled by the event host rather than CCWC, and the conduct of other teams, spectators, and the public.

I confirm that my wrestler is physically fit to participate, has no condition that would make participation unsafe unless disclosed on this form, and that I will keep CCWC informed of any change.

03Voluntary participation and assumption of risk

My wrestler's participation is voluntary. Knowing the risks described in Section 02 and any others that are inherent in wrestling and travel, whether or not listed, I expressly and voluntarily assume all such risks, both known and unknown, on behalf of myself and my wrestler, including risks that arise from the ordinary negligence of CCWC.

04Transportation consent

I understand that CCWC travel typically uses private vehicles, rental vehicles, or passenger vans driven by CCWC coaches, staff, or volunteer parents who are not professional drivers. I consent to my wrestler being transported to, from, and during each Event by drivers CCWC designates, and I agree that:

  • CCWC requires every designated driver to hold a valid driver's license and current automobile insurance and to complete a background screening, but CCWC does not independently verify driving records and is not the insurer of any vehicle. The driver's own automobile insurance is primary for any vehicle incident.
  • My wrestler will wear a seat belt at all times and follow the driver's instructions.
  • My wrestler will not ride with any adult who is not on the Event roster unless I have approved it in writing (a text message to the head coach is sufficient).
  • If I choose to transport my wrestler myself, I remain responsible for my wrestler until I hand them off to a CCWC coach or chaperone at the Event, and again once I pick them up.

05Chaperones, lodging, and supervision

CCWC coaches and adult chaperones will supervise wrestlers during each Event, and CCWC will maintain at least one screened adult for every eight wrestlers traveling without a parent. I understand and agree that:

  • Wrestlers traveling without a parent will share hotel rooms with other wrestlers of the same gender and similar age. No adult will share a room or be alone behind a closed door with a wrestler who is not their own child. Adults check rooms at curfew and lights-out.
  • Supervision is reasonable, not continuous. There will be periods, such as meals, hotel down time, and warm-ups, when my wrestler is not under direct observation.
  • If I attend the Event, I am responsible for my wrestler outside of scheduled team activities.
  • CCWC may end my wrestler's participation and send them home at my expense for conduct that endangers anyone, violates the CCWC Wrestler Code of Conduct, or involves alcohol, vaping, drugs, weapons, hazing, bullying, or leaving the group without permission. I will arrange transportation home within 24 hours of being notified.
  • I will pay for any damage my wrestler causes to hotel rooms, vehicles, venues, or other property.

06Medical authorization

If my wrestler is injured or becomes ill and I cannot be reached promptly, I authorize CCWC coaches and chaperones to obtain and consent to first aid, care by an athletic trainer or event medical staff, emergency transport, and treatment by a licensed physician, hospital, or urgent care, including diagnostic tests, anesthesia, and surgery that a physician considers necessary. CCWC will attempt to reach me before any non-emergency care and as soon as possible after emergency care.

  • I am responsible for all medical costs. I will keep my wrestler covered by health insurance for the season and provide current insurance details below.
  • I authorize event medical staff to perform skin checks and to withhold my wrestler from competition for a suspected communicable skin condition, and I authorize coaches to withhold my wrestler for suspected concussion until cleared by a qualified medical professional.
  • I authorize CCWC to give my wrestler the over-the-counter medications I approve below, in package-directed doses, and to hold and hand over any prescription medication I send in its original container with written directions.
  • I release CCWC and its coaches and chaperones from liability for decisions made in good faith under this authorization.

07Release, waiver of liability, and indemnity

In exchange for CCWC allowing my wrestler to participate in its travel Events, and to the fullest extent permitted by North Carolina law, on behalf of myself, my wrestler, and our heirs, personal representatives, and assigns:

  • I release, waive, and discharge CCWC from any and all claims, demands, causes of action, and liability for personal injury, illness, death, or property damage arising out of or related to my wrestler's participation in any Event, including claims arising from the ordinary negligence of CCWC. This release does not apply to gross negligence, willful or reckless misconduct, or intentional wrongdoing.
  • I agree not to sue CCWC for any claim covered by this release, and I understand that this agreement is intended to be as broad and inclusive as North Carolina law permits.
  • I will indemnify and hold CCWC harmless, including reasonable attorney's fees, from any claim brought by or on behalf of my wrestler, or by a third party, that arises from my wrestler's conduct, from a condition I failed to disclose, or from my own breach of this agreement.
  • This release also extends to the host, venue, and sponsors of each Event to the extent they require it as a condition of my wrestler's entry.

I understand that North Carolina courts have not conclusively decided whether a parent's release of a minor's claims is enforceable, and that CCWC relies on this agreement, on my acceptance of its benefits, and on my promise to indemnify in allowing my wrestler to travel.

08Fees, codes of conduct, and media

  • Fees. Travel Event fees cover registrations, hotels, transportation, and coaching and are set at cost. Because CCWC commits to these costs in advance, fees are non-refundable after the published deposit deadline unless CCWC can fill the spot, and I understand a no-show costs the club real money.
  • Codes of conduct. My wrestler and I agree to follow the CCWC Wrestler Code of Conduct and Parent Code of Conduct, which are incorporated into this agreement, at every Event, in every vehicle, and at every hotel.
  • Placement. Event entries, weight classes, and divisions are the coaches' decision under CCWC's published travel policy (one wrestler per weight per division, wrestle-offs, and coach's discretion).
  • Media. Unless I opt out below, I grant CCWC permission to photograph and record my wrestler at Events and to use those images, with my wrestler's name and results, on CCWC's website, social media, and promotional materials without compensation.

09General terms and electronic signature

  • This agreement is governed by North Carolina law. Any dispute will be brought in the state or federal courts located in Wake County, North Carolina.
  • If any part of this agreement is found unenforceable, the rest remains in effect, and the unenforceable part will be enforced to the greatest extent the law allows.
  • This is the entire agreement about travel-event participation for the season and replaces any earlier travel waiver. It survives the end of the season for any Event that occurred during it.
  • Where two parents or guardians share legal custody, the signing parent confirms they have authority to sign for the wrestler and agrees the release binds both.
  • I consent to signing this agreement electronically under the North Carolina Uniform Electronic Transactions Act and the federal E-SIGN Act. My typed name below is my signature and has the same effect as a handwritten signature. I can request a paper copy at any time.
